Nursing Facility License Renewal Application Changes

The Texas Department of Aging and Disability Services (DADS) began issuing licenses with a three-year effective dates on September 1, 2013. Licenses with three-year durations are set to begin expiring on September 1, 2016. DADS is revising renewal applications for these licenses.

The following parts of Item 11 on Form 3720-N have changed:

• Part 1 was deleted in its entirety; and

• Part 2 was amended to recognize the three-year duration of the license.
