Center for Excellence in Long Term Care: Nursing at Its Best Award

What does it take to be the best nurse or nurse assistant in long-term care? The Center for Excellence in Long Term Care wants to know.

The Center for Excellence in Long Term Care is seeking nominations for healthcare providers at the CNA, LVN, RN, and APRN levels who have provided excellent person-centered care to patients in a long-term care setting. This can be at a nursing home or skilled nursing facility.

Nominations are accepted that fit the following criteria:

  • Work in long-term care setting in Texas
  • Must be at least a year in position
  • Demonstrates past or present person-centered care

Nomination process: Submit 250 words or less on why you think the nominee is deserving of this award. Self-nominations are acceptable. Please use the online submission form found at the bottom of this page. Nominations are accepted throughout the year.

Selection process: This is a quarterly award. There will be three awards given out in 2018. The first submission deadline is March 9, 2018. Nominations submitted after this date will be held and reviewed during the next award cycle.

Winners: A review committee will announce one winner per quarter. Winners will be featured on the Center for Excellence in Long Term Care’s website and social media pages. Each winner will also receive a certificate and gain the opportunity to have the Center for Excellence in Long Term Care administer a free CEU workshop at their facility.
