Dual Demonstration Provider InformationBelow is information that you may find helpful if you are in one of the counties (Bexar, Dallas, El Paso, Harris, Hidalgo, and Tarrant) involved in the Dual Demonstration program: Enrollment – Sample letters that are sent to all dual eligibles in the demonstration county can be found on the HHSC website From the link you can find letters for people who received Medicare and Medicaid who can choose to enroll in a STAR+PLUS Medicare-Medicaid Plan
Address Where Enrollment Letters are Mailed: To change the address with Medicare, the recipient or representative can change the address online www.Yourtexasbenefits.com or call 211. To opt out of the Dual Demonstration the recipient or their representative has to make a telephone call to either Medicare or Maximus. The telephone numbers are 1-800-Medicare or 877-782-6440 for Maximus If your nursing facility is in one of the dual demonstration counties and you do not have an Executed Contract for the MMP program, contact: Amerigroup – Kelley Longhofer klongho Common Authorization Request Form – TDI has a form that the MCOs have to accept beginning September 1.
