Administrators: Please complete the THCA Facility Contact Information Sheet,
(MICROSOFT WORD VERSION – .DOC)
(PORTABLE DOCUMENT FORMAT – .PDF)
and return it to THCA. This will ensure we have your current contact information in our membership database and that of your D.O.N. This will enable you both to receive, as quickly as possible, important information we send electronically to THCA members. This information will also be used to assign you each your personal/unique User Name and Password to access the Members Only information on the THCA Website, www.txhca.org.
